Serenity Home Care Facilities, LLC ("Serenity," "we," "us," or "our") operates two licensed Intermediate Care Facilities in Riverside County, California: Aces House in Lake Elsinore and Papillon DDN in Menifee. This Privacy Policy explains what information we collect through our website, serenityhcf.com (the "Site"), how we use and share it, and the choices you have.

By using the Site, you agree to the practices described in this policy. If you do not agree, please do not use the Site or submit information through it.

This Policy Covers Our Website Only

This policy applies to information collected through the Site, including our contact form and online job application. It does not cover the health and personal information of the people who live in our homes.

Resident health information is protected under federal and California law, including the Health Insurance Portability and Accountability Act (HIPAA) and the California Confidentiality of Medical Information Act (CMIA), as well as the resident rights and confidentiality requirements that apply to licensed facilities and Regional Center services. Residents and their legal representatives receive a separate Notice of Privacy Practices describing how resident health information is used and disclosed and the rights they have. To request a copy, contact us using the information in Section 12.

Please do not send medical details or other private health information through the Site. Our website forms and regular email are not intended for health information. If you are a family member, conservator or Regional Center case manager, submit your contact details and we will call you to talk through care needs.

Information We Collect

Information you give us
  • Contact form: your name, phone number, email address, the type of inquiry (family member or conservator, Regional Center case manager, job seeker, or other), and your message.
  • Online job application: your name, phone number, email address, city and ZIP code; the position and home you are applying for; shift availability and earliest start date; whether you are authorized to work in the United States and are 18 or older; certifications and license numbers; driver's license status; work history; relevant experience; professional references; an optional resume; and your typed name and date as a signature.
  • Email and phone: any information you choose to include when you email or call us.

Our job application does not ask for your Social Security number, date of birth, criminal history or medical information, and we ask that you not include them. If we make you an offer of employment, we will collect any additional information required by law at that time, through our hiring process rather than the Site.

Information about other people: if you give us information about someone else, such as a professional reference or a loved one you are inquiring about, please share only what is necessary and make sure you have the right to share it.

Information collected automatically

Like most websites, the Site and the service providers that host it may automatically collect limited technical information when you visit, such as your IP address, browser and device type, pages viewed, the website that referred you, and the date and time of your visit. This information is used to operate, secure and improve the Site and to prevent spam and abuse of our forms.

Cookies and Similar Technologies

The Site may use cookies or similar technologies that are needed for the Site and its forms to work, including spam protection on our forms. We may also use website analytics tools that use cookies to show us, in aggregate, how visitors use the Site. You can set your browser to refuse or delete cookies; some features of the Site may not work properly if you do.

We do not use cookies or tracking technologies to collect health information, and we do not use them for targeted advertising.

How We Use Your Information

  • To respond to your questions, referrals and requests, including calling you back.
  • To review and process job applications, contact references, and communicate with you about employment opportunities.
  • To send you a confirmation that we received your submission.
  • To operate, maintain, secure and improve the Site, and to detect and prevent spam, fraud and abuse.
  • To comply with legal, licensing and regulatory obligations, and to protect our rights and the safety of our residents, staff and others.

We do not use the information you submit through the Site for marketing to you, and we do not send newsletters or promotional email.

How We Share Your Information

We do not sell your personal information, and we do not share it for cross-context behavioral advertising. We share information only as described below:

  • Service providers that help us run the Site and our business, such as website hosting, form-processing, email, spam-protection and analytics providers. They may use the information only to provide services to us.
  • Hiring-related parties, such as the professional references you list, when we contact them about your application.
  • Legal and regulatory requirements: when required by law, regulation, court order or a licensing or government agency, or to report suspected abuse or neglect as required by law.
  • Protection: when we believe in good faith that disclosure is needed to protect the health, safety, rights or property of our residents, staff, visitors or others.
  • Business transfers: in connection with a merger, sale or transfer of all or part of our business, subject to this policy.

How Long We Keep Information

We keep contact form submissions for as long as needed to respond to you and handle any follow-up. We keep job applications and related records for as long as required by applicable employment law and as needed for our hiring process. When information is no longer needed, we delete it or keep it in a form that no longer identifies you.

How We Protect Information

We use reasonable administrative, technical and physical safeguards designed to protect the information you submit through the Site. However, no website, internet transmission or email is completely secure, and we cannot guarantee the security of information you send to us online. This is one reason we ask you not to send health information through the Site.

Your Choices and Rights

You may contact us at any time to:

  • ask what personal information we have about you from the Site;
  • ask us to correct or update information you submitted;
  • ask us to delete information you submitted, or withdraw a job application; or
  • ask us to stop contacting you.

We will respond within a reasonable time. We may need to verify your identity before acting on a request, and we may keep certain information where the law requires us to or where we need it for legal, licensing or record-keeping purposes. We will not discriminate against you for making a request.

California residents may have additional rights under California law. Contact us using the information in Section 12 if you have questions about them.

Do Not Track Signals

Some browsers offer a "Do Not Track" setting. Because there is no common industry standard for responding to these signals, the Site does not currently respond to them. As stated above, we do not use tracking technologies for targeted advertising.

Children's Privacy

The Site is intended for adults. We do not knowingly collect personal information online from children under 13, and our job application is open only to applicants 18 or older. If you believe a child has submitted information through the Site, contact us and we will delete it.

Links to Other Websites

The Site may link to websites run by others, such as the California Department of Public Health, Inland Regional Center or other agencies and resources. We are not responsible for the privacy practices of those websites. Please review their privacy policies.
